Please note that you will need to provide ONE of the following for each registered child. \n\n\n\n\n\nReport card or progress report from the 2025-2026 school year\nSchool schedule or class schedule from the 2025-2026 school year\nStudent ID card\nSchool enrollment or registration confirmation\nSchool transcript\nAttendance record from 2025-2026\nParent portal screenshot showing the student’s name and school (e.g.\, Infinite Campus\, ParentVUE)\nAcceptance or enrollment letter from the school district-Attestation form from Merrick case manager\nIf you have any questions contact Abigail Hernandez atahernandez@merrickcs.org.
